Development Family Caregiving

The individual receiving services:

• Must be an Arizona Long Term Care (ALTC) participant.

• Must have the services needed approved by AZ Department of Economic Services, Division of Developmental Disabilities. (DDD)

• Both primary caregiver and their choice of family member must participate in a phone and in person meeting to determine if we are ideal for your needs.

• Must be a HARC Member with the required services provided by HARC Human Services.

• Must complete all documents required by HARC, DDD and the State of Arizona prior to the first activity.

The chosen caregiver:

• 18 years old

• Must be a family member

• Able to pass background check

• Able to obtain Level one Fingerprint Card

• Must be a US Citizen, or proof able to work in the US

• Driver’s license, proof of current registration and insurance if transporting
