Must be an Arizona Long Term Care (ALTC) participant.

Must have Respite Care services approved by AZ Department of Economic Services, Division of Developmental Disabilities. (DDD)

Must participate in a phone and in person meeting to determine if we are a good fit for your respite needs.

Must be a HARC Member with Respite Care services through HARC Human Services.

Must complete all documents required by HARC, DDD and the State of Arizona prior to the first activity.

Must use the online portal to sign up for an activity and meet the activity’s specific requirements before the close date. In order to provide an enjoyable, safe outing for your loved one, we will provide a maximum participant count for each activity.
